The Canada launch registration uses an extended form. It includes all fields from the standard country launch registration, plus additional sections for **Company information** (company ID, company name, DBA name, company website, legal entity type, stock symbol and exchange if publicly traded, company overview, and industry sector), a **Company address** (address lines, city, state/province, postal code, and country code), **Extended contact information** (a primary contact phone number in addition to the baseline contact details), and **RCS partner information** (RCS partner name, contact name, and contact email).
